(If you want to remove any words or numbers please state that in the notes)
(If you want the shirt to be the colors in the picture please state that in the notes)

- First name for hood (one name only)
- Last name (one name only)
- Number (one player number only)
- Design Colors (Two colors only/color chart in photos)
